Medical Social Service
Physical therapy
At Agave Home Health, the primary goal of our therapists is to enhance the adaptability of patients to their home environment and to help them achieve the highest lifestyle functionality given the limitations of their medical or physical conditions. When requested by the doctor, the Physical Therapist provides and educates the patient in self-care, mobility, safety, range of motion, and strength activities.
Physical therapists develop programs to restore function, maximum mobility, reduce pain, and to prevent disability following disease, injury, or loss of a body part.
Our Physical Therapy program involves:
- An individualized plan of care based on the patient’s needs
- Plan of care is directed by the patient’s physician
- Improvement in mobility after surgery, injury, illness, etc
- Pain management in acute and/or chronic situations
- Improvement in strength and endurance.

Occupational Therapy
At Agave Home Health, Our Occupational Therapists evaluate and develop treatment plans to restore and improve our patient’s independence in performing activities of daily living such as: bathing, dressing, transfers, housekeeping, etc. The Occupational Therapists assist patients in improving and restoring lost and/or impaired functions, and to adapt to functional limitations affecting the activities of daily living.
Our Occupational Therapy program involves:
- An individualized plan of care based on the patient’s needs
- Plan of care is directed by the patient’s physician
- Improvement in muscle strength and functional range of motion
- Improvement in mobility, independence, balance, etc
- Assistance with the use of adaptive devices

Speech Pathology
The ability to communicate is a very important aspect of our lives. At Agave Home Health, the speech therapist will assist you in regaining essential communication skills. The speech therapists will also assist patients with feeding or swallowing problems. They also educate patients on ways to improve their memory.
Our Speech Therapists provide services for patients requiring:
- Improving their ability to produce words
- Developing expressive language
- Increasing vocabulary and developing articulation

The Wound Care Management Program
Wound care has become one of the most expensive areas of home care services and one of the highest users of home visits. It is important that home care clinicians have a good understanding of wound care, the healing process, products and product usage, as well as appropriate care and treatment modalities.
At Agave Home Health, our wound care program provides a multidisciplinary approach that combines the skills of RN nurses, physical therapists, and dieticians as needed. Our specialty RNs have experience with comprehensive wound assessments and wound care using the latest wound care techniques.
In general our wound care management program includes the following:
- Specialized care to patients with skin and wound problems
- Care is provided/managed by wound care RNs
- The program utilizes clinical pathways/DSM protocols as necessary

Pain Management
The Pain Management Program
Pain is a complex, subjective, and unpleasant sensation that is very common with the elderly. Chronic pain can lead to social isolation, depression, polypharmacy, decreased functional status, and deconditioning.
As such, pain management is a critical aspect of geriatric care. The elderly are generally at increased risk of adverse effects from drug interactions due to the use of analgesics and other pain killing medications. As a result non-drug treatments that reduce pain are of particular importance. Patient and caregiver education and active patient/caregiver involvement can help reduce pain and improve quality of life of the patient.

Colostomy/Ostomy Care
A colostomy is an opening in the belly (abdominal wall) that is made during surgery. The end of the colon is brought through this opening to form a stoma. Where the stoma will be on the abdomen depends on which part of the colon is used to make it. The enterostomal therapy nurse (ET nurse) or the surgeon will figure out the best location for your stoma.
Colostomy surgery is done for many different diseases and problems. Some colostomies are done because of malignancy (cancer), others are not. Sometimes a colostomy is only needed for a short time, sometimes it is life-long. Some colostomies are large, some small; some are on the left side of the abdomen, some are on the right side, others may be in the middle.
A colostomy is not a disease, but a change in the way your body works. It surgically changes normal body function to allow stool to pass after a disease or injury. Although a colostomy is a big change for the patient, the operation itself is rather simple. The body’s chemistry and digestive function are not changed by having a colostomy.
